| Register: Children with Special Health Concerns
Every effort to accommodate children with special health concerns is made by the AFTER SCHOOL staff. To ensure that the needs of all the children in our programs are met, all Health History forms that indicate that there are Special Health Concerns are reviewed prior to enrollment. Our review is based on the individual needs of each individual child; at all times the goal is safe and successful inclusion in the AFTER SCHOOL environment.
The process generally includes a phone call from a Program or Community Director to the parent to gather additional information that will assist in providing an environment that will enable the child to be safe and successful in the AFTER SCHOOL program. For returning participants, these calls help update our records and provide an opportunity to share any changes to the special health concerns your child is experiencing. Additional information gathered will be added to your child's registration materials. Any needed modification will be discussed and documented to insure complete communication with the on-site program staff. On an as needed basis, some forms are shared with the Division Director or Risk Management Director for further consult and recommendation.
At times a child needs medication during the program hours on a temporary or regular basis, may need to check blood glucose levels or may need an emergency medication such as an inhaler or epi pen. The required forms will be discussed during the phone call and sent to the parents for completion. You may download the Authorization to Administer Medication form here, and view samples of the addendum forms with the following links:

While this process may delay processing your child's enrollment, in addition to information gathering and establishment of needed modifications, one main objective of this process is to establish a foundation of communication with parents regarding their child that has special health concerns. If you have a child with special health concerns, please send your registration early to avoid delays beyond the time you would like to begin accessing the program. |
